The county’s Primary Care Connection: Employee Health and Wellness Center is open at 1939 Salk Ave., Tavares. Pre-scheduled appointments eliminate long wait times, and are available by calling 1-866-959-9355.
The Primary Care Connection (PCC), operated in partnership with Healthstat, is staffed with a doctor and medical assistant who are able to diagnose and treat a variety of ailments, test vital signs, conduct blood draws and prescribe and dispense common medications for anyone under the Lake County health plan, including dependents, free of charge.
- Disease management such as diabetes, cholesterol and blood pressure
- Physicals for those ages six and up
- Flu shots
- Lifestyle and health coaching for those wishing to quit smoking or lose weight
Early detection of potential health risks improve quality of life, plus using the center as a primary care service is expected to result in both savings for the county and its employees under the self-funded medical plan.
